Athletic Physicals

Physical forms can be found in your school’s activities office. Completed physicals must be uploaded into rSchools.

These are sports physicals and do NOT take the place of a comprehensive physical. No immunizations will be given.

Beginning July 1, 2019, sports physicals and concussion forms will need to be uploaded into a website called rSchools rather than being turned into the activities office.

The forms must be filled out and uploaded before a student can participate in athletics.

Before you begin the registration process, you will need to have electronic copies of your student’s completed physical form. If you do not have access to a scanner, you may bring your forms into the activities office and a scanner will be made available to you so you or your student may email the forms to yourself. The activities office does not have the capability of uploading forms into rSchools for you.

Sports Physicals Offered Locally

Students participating in sport competitions are required to have a physical examination so the student may safely engage in athletic competition. An Iowa Athletic Pre-Participation Physical Examination Form must be completed and uploaded in rSchools before a student can participate in athletics.
